Wellness Plan Healthy Activities deadline extended!

The State has extended the 2015 Wellness Plan healthy activities deadline to December 31, 2015. This means that participants now have an additional 90 days to complete the following activities for 2015:

  1. Select a Primary Care Physician (PCP)
  2. Complete the health risk assessment; and
  3. Review the results of the health risk assessment with their PCP.

For those individuals not completing the activities by December, the $50 surcharge will be delayed until April.  Details will be available on the DBM website and the medical carriers' websites.

Those deadlines are as follows:

The 9/30/15 deadline for Wellness Plan questionnaires to be completed has been moved to 12/31/15.

The 1/1/15 $50 surcharge for choosing not to name a primary care physician (PCP), complete questionnaire & discuss w/ PCP has been moved to the first pay period in April 2016.
